Dance Team contract
2025-09-15 Between: Dancer and Studio X Ottawa Inc. for Dance Teams 2025-2026.
Contract Start Date: 2025-09-15
Contract End Date: 2026-09-30
Contract Duration and Automatic Renewal: Contract is in force for 11 months from the Contract Start Date.
Additional practice may be added before important performances (at no extra cost).
Agreement to Pay Recurring Fees: The dancer agrees to pay Studio X Ottawa Inc. monthly recurring fees as follows:
Dance Teams 2024-2025 for $100 + HST via Client authorized automatic credit card.
Payment in monthly increments by any means other than Client’s credit card is not permitted.
Team members obligation, they will:
- Pay for their costume, traveling expenses (hotels, dancers pass, etc.) and any other additional costs (ex. hotel room, travel cost, etc). * Some of those costs might be share with other team members.
- Help teaching at workshops (ex. local and out of town).
- Help during socials once in a while (if available).
Team members benefits:
- Rebate on entrance fees to our monthly social starting at the end of October 2025.
- 50% rebate on private classes (if needed).
- Access to studio to practice the routine on your own or with other team members when the Studio is available

(a) I agree to purchase the Dance Teams 2025-2026 for $100 + HST, as an automatic charge to my credit card each month for a contract period of 11 months.
(b) I hereby certify that I am the holder of the credit card detailed below.
(c) I understand that I will be notified if my credit card or checking account payment fails to authorize for any reason, and that a $25 late fee will apply if I do not provide a valid credit card within 10 calendar days of the original rejection date.
(d) I understand that my service will be deactivated if my account becomes more than 30 calendar days late.
Rules: You agree to follow all instructions of your coaches.
Health: You represent that you do not have any health condition that strenuous exercise could make worse. You understand that our coaches are not medically trained to monitor your health during exercise. You agree to contact your own doctor for any questions regarding your ability to exercise safely. This program is for your recreational use only and nothing stated, given, or written should be taken as medical advice.
Early Termination Fee: You may cancel this agreement before the agreed upon term has been reached by paying the remaining balance left on the agreement. For the people on probation, this contract.
*For the people on probation, this contract can be terminated right after the probation period. After that, all the terms and conditions stated in this contract will apply to them as well.
Media Release
I understand and accept that my image, comments, likeness, accomplishments, goals, and challenges I overcame may be used by Studio X Ottawa Inc. and that I am not entitled to any compensation for such use. I reserve the right to have the aforementioned remain private by submitting such a request within 15 business days of signing this agreement. I understand that my image and voice may be recorded/captured during calls, online sessions, and in person.
By signing you acknowledge you have read, understand, and agree to all terms on the entire agreement.
